Laboratory Testing Policy

In certain situations, providers may require laboratory testing as part of ongoing treatment, medication monitoring, or patient safety.

Please note:

1. Laboratory testing costs are separate from office visit charges.
2. Patients are responsible for contacting the laboratory directly regarding pricing, insurance coverage, and billing questions.
3. Patients may choose the laboratory facility of their preference unless otherwise required by insurance or clinical necessity.
4. Failure to complete requested laboratory testing may affect the provider’s ability to continue treatment or prescribe certain medications.
