Pinnacle Psychiatry may use and disclose your Protected Health Information (PHI) for purposes related to treatment, payment, healthcare operations, and other uses permitted or required by law.

Patients have the right to:

1. Request access to or copies of their medical records, subject to legal limitations
2. Request restrictions on certain uses or disclosures of PHI
3. Request confidential communication through alternative methods or locations
4. Request amendments to portions of the medical record
5. Obtain an accounting of certain disclosures of PHI
6. Receive a paper copy of this Notice of Privacy Practices upon request
