Paperwork, Forms & Administrative Requests

Requests for paperwork, forms, letters, FMLA/disability documents, emotional support animal (ESA) letters, school/work accommodations, or other administrative documentation may require additional review time and may be subject to administrative fees.

Please note:

1. FMLA paperwork, disability paperwork, and similar extended leave documentation will not be completed for new patients.
2. Patients must be established with the practice for a minimum of 6 months before such requests will be considered.
3. Completion of paperwork is not guaranteed and remains at the sole discretion of the provider based on clinical judgment and appropriateness.
4. Letters for school or workplace accommodations may be provided upon request when clinically appropriate.
5. Forms and documentation requests may require a scheduled appointment prior to completion.
6. Processing times may vary depending on complexity and provider availability.
7. Expedited or same-day completion requests may not be possible.

Patients are encouraged to submit paperwork requests well in advance of deadlines.
