Controlled Substance Policy
Controlled substance prescriptions are provided at the sole discretion of the treating provider and only when clinically appropriate.
Patients receiving controlled substances may be required to:
- Attend regular follow-up appointments
- Successfully complete urine drug screenings with results consistent with prescribed treatment and provider expectations
- Provide specialist clearances if medically indicated
- Complete additional monitoring requirements
- Use preferred or approved pharmacies based on insurance requirements
Failure to comply with treatment recommendations, monitoring requirements, follow-up visits, or requested documentation may affect the provider’s ability to continue prescribing medications.
Early refills, replacement prescriptions for lost/stolen medications, or repeated missed follow-up visits may not be approved.
