Controlled Substance Policy

Controlled substance prescriptions are provided at the sole discretion of the treating provider and only when clinically appropriate.

Patients receiving controlled substances may be required to:

  1. Attend regular follow-up appointments
  2. Successfully complete urine drug screenings with results consistent with prescribed treatment and provider expectations
  3. Provide specialist clearances if medically indicated
  4. Complete additional monitoring requirements
  5. Use preferred or approved pharmacies based on insurance requirements

Failure to comply with treatment recommendations, monitoring requirements, follow-up visits, or requested documentation may affect the provider’s ability to continue prescribing medications.

Early refills, replacement prescriptions for lost/stolen medications, or repeated missed follow-up visits may not be approved.
