Insurance Verification & Eligibility
As a courtesy, our office may verify insurance eligibility and benefits prior to appointments; however, insurance verification is not a guarantee of payment.
Patients remain financially responsible for:
- Copays
- Deductibles
- Coinsurance
- Non-covered services
- Claims denied by insurance
- Balances applied toward deductibles or out-of-pocket maximums
If insurance coverage is inactive, invalid, or incomplete prior to an appointment, updated insurance information must be provided promptly in order to maintain the scheduled visit.
Patients are responsible for understanding their insurance benefits, network status, referral requirements, and pharmacy restrictions.
